Update (28.05.2014):
In the last day, I have added a new version of this exploit to the original version, which lets you quickly calculate the required CIFs for the five Nokia Lumia and/or Nokia Windows Phone models.
The tool is now completely offline, including the delivery of the lock screen. The only thing you need to do is to install the application to your computer and connect it to your phone. The application opens up in a separate window (Nokia only in version 5.0).
Download the utility, unzip it and double click on the exe to run the program. The required parameters can be quickly and easily entered. In case you are curious, here are the available Nokia models:
